Short-term rehabilitation is also known as post-acute rehab. The short-term rehabilitation programs at Sunharbor Manor help our patients achieve their maximum functional capacity and get back to their homes and community in the shortest time possible. Rehabilitation residents have access to the same activities and amenities that our full-time residents enjoy.
Patients receive physical, occupational and speech therapy from compassionate and highly skilled therapists:
- Physical therapy: Our PT professionals incorporate a variety of exercises to promote better movement, balance, and strength. We can treat a wide variety of movement disorders.
- Occupational therapy: In our fully equipped rehabilitation gym, we focus on restoring motor coordination, manual dexterity, and sensory perception. Our goal is to improve your mobility and help you maintain an active lifestyle.
- Speech therapy: We help patients regain language skills and overcome swallowing problems that are common among stroke survivors, those with speech disorders, and people with other health conditions that affect speech.
The physical therapists, occupational therapist, and speech therapists are part of a team that includes physicians, nurses, social workers and nutritionists. Together, they work with the patient and family members to develop an individualized care plan. Every member of our clinical team and residential staff is meticulously trained in the art of creating and maintaining real relationships with residents and families, forming special bonds that transcend medicine.
Our rehabilitation team includes the following specialty staff:
Rehab Services
- Physiatry: Every short-term rehab stay patient will be seen and assessed by a physiatrist upon admission. Physiatry will work closely with the rehab team to ensure your pain management and rehab needs will be met.
- Return-to-work program: For those short-term stay patients who plan to return to work, a customized rehab plan will be designed especially for you and your needs. You will be back to work in no time.
- In-home assessment: During your rehab stay, your therapist will visit your home to assess how we can accommodate your needs upon discharge. Taken into consideration will be stairs (should you have), durable medical equipment (if you require), activities of daily living (ADLs), and any other medical equipment you or your loved ones are advised to install in order to ensure your safety and quality of life. (This is done on a case-by-case basis.)
- Rehab aides: Our rehab aides are there to provide our residents with range of motion, bed mobility, standing and floor ambulation on the units.
- Occupational therapists: Our occupational therapists do rounds during mealtimes to help patients with eating when needed.
- Speech pathology: Our speech therapist utilizes a fiber-optic swallow evaluation for those cases that should require a special assessment.
