𝟭/𝟭𝟰/𝟮𝟮 𝗡𝗬𝗦 𝗠𝗔𝗡𝗗𝗔𝗧𝗢𝗥𝗬 𝗩𝗜𝗦𝗜𝗧𝗔𝗧𝗜𝗢𝗡 𝗧𝗘𝗦𝗧𝗜𝗡𝗚 𝗨𝗣𝗗𝗔𝗧𝗘
The NYS Department of Health has instituted new COVID-19 testing requirements for all Nursing Home visitors. Due to the increase in prevalence of the Omicron variant in the State, Visitors must now be tested for COVID-19 prior to visiting a facility.
EACH visitor must receive either:
- A negative rapid antigen test result conducted no more than 𝐨𝐧𝐞 𝐝𝐚𝐲 prior to their visit, or
- A negative NAAT (e.g. PCR) test result conducted no more than 𝐭𝐰𝐨 𝐝𝐚𝐲 prior to the visit.
This means, for example, that a test for a Sunday visit should be conducted no earlier than Saturday if it’s an antigen test, or Friday if it’s a PCR test.
If a visitor cannot provide a valid negative test result on entry, they will be required to test themselves before they are allowed to visit. Visitors may be provided a rapid antigen test, educated on its use, and instructed to test themselves in their car/outside the facility and wait for the negative result before reentering the building. The facility must review that negative test on reentry, but will not certify the result.𝐈𝐟 𝐚 𝐭𝐞𝐬𝐭 𝐫𝐞𝐬𝐮𝐥𝐭 𝐢𝐬 𝐩𝐨𝐬𝐢𝐭𝐢𝐯𝐞, 𝐲𝐨𝐮 𝐦𝐮𝐬𝐭 𝐍𝐎𝐓 𝐞𝐧𝐭𝐞𝐫 / 𝐫𝐞𝐞𝐧𝐭𝐞𝐫 𝐭𝐡𝐞 𝐟𝐚𝐜𝐢𝐥𝐢𝐭𝐲 𝐮𝐧𝐝𝐞𝐫 𝐀𝐍𝐘 𝐜𝐢𝐫𝐜𝐮𝐦𝐬𝐭𝐚𝐧𝐜𝐞𝐬.
Visitors are advised to follow all applicable public health guidelines and contact their medical provider following a positive COVID-19 test result.
𝐕𝐢𝐬𝐢𝐭𝐨𝐫𝐬 𝐰𝐡𝐨 𝐯𝐢𝐬𝐢𝐭 𝐟𝐨𝐫 𝐦𝐮𝐥𝐭𝐢𝐩𝐥𝐞 𝐝𝐚𝐲𝐬
Visitors who visit for multiple days must provide proof of negative testing as often as feasible, at a minimum every third day (meaning at a minimum testing is required on day 1, day 4, day 7, and so on).
𝐕𝐢𝐬𝐢𝐭𝐨𝐫 𝐦𝐚𝐬𝐤 𝐮𝐩𝐝𝐚𝐭𝐞
- Visitor mask update – As of 1/12/22, all visitors must wear a paper/surgical mask or higher quality mask (KN95, N95) while in the building. Cloth masks cannot be used as a primary mask anymore in NY Nursing Homes, but may be worn over a paper mask, if desired.
- Surgical masks are provided for all visitors on entry as needed. Your mask must be worn over both the nose and mouth, and all visitors should continue to physically distance from other facility personnel/patients/residents/visitors that are not directly associated with your visit.
As always, visitors who test positive after visiting must notify the facility immediately. Visitors should not visit if they develop new symptoms of potential COVID-19, even if they previously tested negative.
*Vaccination Reminder – All residents who are eligible to receive the Covid-19 vaccine or booster may do so at the facility. Residents and visitors are strongly encouraged to be vaccinated or receive a booster (if eligible). Please reach out to the Nursing or Social Work department.

VACCINATION BOOSTER UPDATE 4/4/22
We are now able to provide the 4th dose of the Pfizer Covid-19 vaccine to eligible residents and staff. Individuals who received their first booster at least 4 months ago may receive their 4th dose here at Sunharbor on our normal vaccination clinic schedule (Next Clinic Wednesday, April 13th).
Eligible residents and staff may receive any U.S. authorized COVID-19 vaccine dose at the facility at our regularly scheduled clinics every other Wednesday (April 13th, April 27th, etc). Please notify our social work department of any residents interested in receiving Covid-19 vaccination here at Sunharbor.
At this time our pharmacy partner is only providing Pfizer vaccination for unvaccinated individuals. Partially-vaccinated individuals who received the Moderna 1st dose prior to admission may receive their 2nd dose with us as needed.

- DO NOT visit if you have traveled to any of the high risk areas for COVID-19 in the past month.

- North/South Korea
- Iran
- Italy
- (Additional areas may be added at any time based on CDC recommendations).
- DO NOT visit if you have been exposed to a person suspected of or confirmed with COVID-19.

Dear Sunharbor families,
as we draw near to the holiday’s, a time historically spent with family units and friends, we now find that current inability to be another change from the norm we once knew. With that in mind, our facility staff continues to do its part to help maintain a holiday cheer. Although we can’t conduct large group activities while our facility experiences Covid-19 cases, our musical minstrels continue to sound in the unit hallways and plenty of individual activities take place as well. Passover booklets and Haggadah pamphlets are being distributed through our Recreation department and On Easter Sunday we’ll be streaming Easter Mass to our resident’s room TV’s for their convenience.
Of course, various foods commemorative of these holidays will be circulating too!
We continue to take all necessary steps under the guidance set forth by the CDC and DOH to care for the Sunharbor residents and staff, and do our part to also “flatten the curve”.

We want to once again acknowledge the dedicated healthcare workers that are doing their part daily to help our residents. Our facility has been aggressively tracking and coordinating efforts to mitigate the risks of Covid-19 within the facility. On 3/26/20 our facility was notified of resident confirmed Covid-19, positive test results.
Upon learning of these results, Sunharbor Manor contacted both state and local health department officials immediately and continues to work collaboratively under the guidance of the New York State DOH.
All necessary precautions have been put in place to continue safe guarding our in-house population.
The Centers for Disease Control and Prevention (CDC) set forth a variety of steps that are implemented to protect our residents and staff. These recommendations include among other things, strict infection control and prevention initiatives, stringent handwashing procedures, restricting visitation and screening all individuals, including staff that are permitted to enter our center.
Additionally, all health care workers adhere to the guidance issued by the CDC and the DOH for transmission-based precautions in caring for our patients.
We have been preparing for such an event and commend the dedicated staff for the wonderful job they are doing.

Although the facility presently has no positive cases in the facility at this time, we’re notifying our Sunharbor community that we’ve been made aware that a resident of the facility tested positive for COVID-19 while outside our facility at a hospital.
All necessary precautions have been put in place to continue safe guarding our in-house population. Although we’ve modified programs and have stopped all large group gatherings, we continue to be present and ensuring the quality and spirit in the operation.
Upon notification by the hospital of this patient’s condition, our facility made contact with local County and State agencies as required, and we’re following the guidance set forth for enhanced infection prevention.
In this ever changing fast paced climate we want to also make special mention to the incredible and dedicated staff we have who are tirelessly keeping up with all that’s required of them and continue keeping our Sunharbor population smiling.

NO VISITORS
The health and safety of our community continues to be our top priority, in the wake of the evolving situation around the novel coronavirus (COVID-19).
Please be advised that after 7 p.m. on Thursday, March 12, 2020, visitation will be restricted/closed to all visitors. Visitors will only be allowed on the Nursing Facility’s floors, if their loved one is on hospice or similar urgency.
Visitors will be restricted from our facility until further notice.
Our Administration and clinical team will reevaluate and advise of any changes at least weekly.
Updates will be posted on the facility website as needed.
